This online form enables you to complete the financial assessment form and attach any supporting documents. We will use this information to decide if we can help pay for your care and support and work out how much.

Before you start

You should make sure you have all the relevant documents and your National Insurance number ready before you start to complete this form. Read our guidance on paying for your care and support (pdf, 4.4 MB) for more information.

You can complete the form for yourself, or on behalf of someone else.

Direct Debit mandate

We will still need you to download print and sign the Direct Debit mandate and return this to us as your bank will need the original document to set this up.

How to complete the form

Make sure you are not using a public computer to complete the financial assessment as it is a security risk that may result in your personal and financial details being exposed to the public.

While you are filling out the online form and do need to get away from the computer for a minute or two, that is fine. You will still be able to continue. However, please don’t leave the uncompleted form or leave it without activities for more than 20 minutes or you will lose what you have entered and will need to start over again.

When you have completed the portal form, please make sure you have hit the submit button. You will then be given an option to print/download a copy of your completed form (highly recommended). Please print right after you have submitted the form as you will not be able to print/ download it afterwards.

What you will need

Before you begin to complete this form, you will need the following:

  • Copies of your bank statements
  • Your National Insurance number
  • Details of any income, assets, and pension you receive
  • The completed Direct Debit form ready to the online financial assessment
  • A valid email address so we can send you an email notification with a copy of the completed form
  • If you are completing this form on behalf of someone else, you will need to know their personal details (this includes their address and contact details) and their consent to contact us on their behalf.
