Fitness suite health statement and commitment

I understand that I am not authorised to use any fitness suites until I have undertaken an induction and completed a pre-exercise questionnaire and that use of the equipment thereafter must be done in accordance with instructions received during induction.

I understand that if I misuse equipment, or do not exercise safely and sensibly within my own body’s limits, my actions could result in harm to myself or others. To avoid this, I will always follow instructions on how to safely use equipment or facilities. Furthermore, I understand that I must prioritise taking a sensible approach to my exercise regime and ensure that I take personal responsibility for tailoring it to my current health and fitness levels.

I accept that it is my responsibility to obtain instructions from the fitness staff on the correct use of any equipment I am unfamiliar with, equipment that is not currently available at the centre, which should become available after my induction or which I should wish to use in other centres.

Should new equipment become available I will not attempt to use this equipment until staff have demonstrated its correct use to me and I will not carry out any activities which I have been told are not suitable for me.

I understand that it is helpful to pass on any relevant information regarding my health and if there are any changes to my health that it is in my best interests to inform a member of staff and complete a new pre-exercise questionnaire (Par Q). I understand I have a right to refuse providing this information, or can provide this information but then withdraw my consent to the London Borough of Richmond upon Thames holding this health-related information at any time, and if I wish to do so I should email leisure@richmond.gov.uk. I understand however that without up-to-date health information Council sports staff will be limited in their ability to offer appropriate advice. I am aware that I can review the Council’s privacy policy. The privacy policy contains information regarding how my personal data will be stored and managed by the Council.

I agree to adhere by the individual centre’s code of conduct. I understand that the London Borough of Richmond upon Thames cannot accept any liability for loss of property or injuries sustained whilst on the premises unless such liability shall arise as a result of negligence or the actions of London Borough of Richmond upon Thames.

If I feel unwell whilst using equipment or facilities, I will immediately notify a member of staff who will notify the first aid personnel to assist.

The London Borough of Richmond upon Thames will endeavour to maintain a safe environment for all to exercise and will ensure their staff are qualified to fitness industry standards.

I understand that if I do not consent to this health statement, then personalised advice, programme and support cannot be provided.
