Our self-appraisal is designed to help you think about your situation and consider what you need support with.
You should think about what works well in your life, how you currently manage day to day and what you would like to change. It may be helpful to do this with a family member, friend or someone else you trust.
You can complete the form for yourself, or on behalf of someone else.
At the end of your self-appraisal you will be given a choice whether you want to submit the information to us or not.
If you submit the information to us we will begin our assessment process, which means someone will contact you about the information you have given.
If you do not want to submit the information to us, you can instead save a copy of your completed self-appraisal and use this information to make your own arrangements.
