My review meeting

My parents and a friend came to my review meeting with me. Other people I know were there too. We talked about what I want to do and what help I will need when I leave school. After the meeting what we said was made into a plan.

Year 11 review

When I was 16 years old an adult care manager came to my year 11 school review meeting. My care manager helped my family and me fill in a form about what I can do and what help I need. This form helped Adult Services to work what money I might have to spend on my housing, support and care as an adult.
