What is continuing care?
Continuing NHS Care is the care given to people who do not need acute hospital care but nevertheless have health requirements, which may go on needing specialist care, regular treatment or other long term support.
For example, someone might need:
- Complex or intensive help from a doctor, nurse or other trained health professional and/or
- Treatment which is needed frequently but cannot be predicted, and/or
- Care if they have a terminal illness.
