What is exercise referral?
A referral pathway from GPs, physiotherapists and other health professionals to local leisure facilities for regular physical activity.
Who qualifies for exercise referral?
Patients must be inactive (less than 30 minutes of moderate intensity activity a week) and have a health condition that could be improved with regular physical activity. Conditions for which patients can be referred include:
- weight management
- diabetes type I and II
- high blood pressure
- raised cholesterol
- osteoporosis
- depression
- stress
- arthritis
- poor balance and mobility
- lower back pain
How do I get started?
Once you have a completed referral form from a health professional and an information leaflet, you need to contact one of the leisure facilities offering exercise referral. If your GP or other health professional does not have a form you can contact the Health Improvement Team.
Is it safe for me to exercise?
A trained fitness professional will provide a full fitness assessment and 12 week programme of activity to meet your individual medical and fitness needs. Continued support will be given throughout the 12 week programme.
Which leisure facilities offer exercise referral?
