The care assessment process

The assessment process should be centred on your concerns and needs and you should be able to discuss how you think these can best be met.

Contact assessment

The first assessment is called the contact assessment. Basic information, such as your name, address, next of kin and the name of your GP and or carer may also be collected at this time.

Overview assessment

The overview assessment may take place at the same time as the contact assessment or at a later date. The overview assessment is much more detailed and during this you will discuss in detail the problems you are experiencing and identify the services that may be needed.

The overview assessment will also explore your previous medical history and social care. You may be asked to consent to share information about your needs with other health and social care professionals if we have identified a service need with you.

Specialist and comprehensive assessments

If we have identified during the overview assessment that you need specialist support you may have another assessment called a specialist assessment with an occupational therapist; physiotherapist or district nurse.

If you have many and complicated needs, you may have a comprehensive assessment. This would involve a variety of professionals who will work together over a period of time to ensure that your package of care incorporates all of your health and social care needs (for example: housing, day care or medical or surgical intervention).

Care plan

As a result of your assessment we will develop a care plan. You will keep this as part of a User Held Care Record, which you will be asked to share with all the health and social care professionals who support you.

Voluntary and other care

If we are unable to provide you with support because you do not meet our eligibility criteria, we will do our best to put you in touch with other organisations or support groups who may be able to help you.

If you have a specific advice need or interest you may wish to search the InfoRich database to find out if there is a support group in the area. You can also contact Richmond CVS for details of local charities and voluntary groups.
