How we decide if you are eligible for a service
To receive any social care service you will first be given a care assessment. Adult care assessment may be undertaken by one of our social workers, care managers or community care assessors, but the first (contact) assessment may also be done by a district nurse or other qualified health care professional. The assessment will usually take place in your home, and you are allowed to have a friend or relative with you during the assessment. If English is not your first language, you can ask for a translator to be there during your assessment.
The pages in this section explain the assessment process and also contain details of the Department of Health's 'Fair Access to Care' guidelines we follow throughout the assessment process
